Tenma, who fell into a trap set by "Johan", is now wanted by the police as the "serial killer".
While evading narrowly the police dragnet, he pursues after "Johan" to prevent another murder and to prove his own innocence. He edges closer up toward the Monster by tracing his secret past.
Nina has ended up being caught by the Neo-Nazi group. Meanwhile, Tenma, looking for Nina, the only person who can tell what "Johan" is, finds out a horrific fact. The ultrarightists are conspiring to set "Johan" up as a second Hitler.

A woman lived there with twin kids some ten years ago. Tenma learns that from neighbors. They also tell him about the Czechoslovakian secret police and a questionable fire. Could the twins be Johan and Anna??
Meanwhile, Suk, a young detective of the Prague Police, gets attracted to a beautiful girl he met in a bar. Her name is Anna! When Tenma and Suk meet, another terror creeps up on them!!
The BKA chief inspector Lunge is now convinced of Johan's involvement in the serial murders. He comes to Prague to find out what Johan really is,
and gets at a fact that "The Nameless Monster", the uncanny picture story book which seems to have deeply affected Johan in his childhood, was created by an agent of the Czechoslovakian secret police.
Lunge manages to locate "the Red rose Mansion" which the author used for writing. And there, in a thickly plastered wall, he discovers an old door!! Now come out something of real horror from behind it!!
